Everything you need to know to start your healing journey.

For your initial appointment (whether Massage Therapy or Physiotherapy) you will need to fill out our Patient Intake forms. You are welcome to fill them out on our website and email them back to us or you can download, print and fill out the forms at home. If you prefer to fill them out at the office, please arrive 15 minutes prior to your appointment to ensure you have enough time to complete the forms and begin your session on time.

Note: On the second page under Current Condition(s) the diagram cannot be filled out online. This part can be completed at the office prior to your session.

Instead of printing off the form above, you are welcome to email it to our office. Please make sure you attach the filled out PDF and provide your full name.

  2. Find the file in your Downloads folder and open with Adobe Reader (not in your browser*)
  3. Fill out the form and Save your information
  4. Email the completed form back to us

    *If you do not have Adobe Reader, you can fill out the PDF in your Browser. However, you will need to follow these steps after filling out your information:

Motor Vehicle Accident (MVA) Injuries & Work Place Related Injuries (WSIB)

If your injuries are a result of a car accident or a work place accident, for your initial appointment please arrive 20-30 minutes prior to your session to ensure you have adequate time to complete all the required paper work and begin your session on time.

If available, please also bring the following:

Motor Vehicle Accident (MVA) Injuries

  • your claim/policy number
  • your adjustor’s name and phone number
  • any forms you received from your Auto Insurer

Work Place Related Injuries (WSIB)

  • your claim number
  • your case manager’s/nurse practitioner’s name and phone number
  • any forms you received from WSIB

Cancellation Policy

In order to be respectful of other patients and in consideration for our therapists, we require 24 hours notice to be given for any cancelled or rescheduled appointments. Failure to provide notice may result in the full fee for your session being charged.

We understand that unanticipated events happen occasionally, therefore extenuating circumstances will be taken into consideration.

RMT Update

At the current time, we are not accepting any new massage therapy patients.

If you have been referred to our massage therapist, please call (519-208-2352) or email ( the clinic to discuss booking as any online booking for a new patient may not be accepted.
